Category 1: Cash & A/R (how fast cash flows in)

Net collection rate (benchmark: 95% or higher). Payments collected as a percentage of allowed amounts, calculated against contracted rates rather than charges. A primary care entity should have a lower days-in-A/R target (closer to 30 days) than an orthopedic surgery entity (closer to 45 days), because procedural billing complexity drives longer collection cycles. A newly acquired entity in the first 90 days post-close should have wider tolerances on most KPIs because integration noise temporarily affects performance.
